Register with SDS
The SDS office is responsible for providing reasonable accommodations for those persons with disabilities on campus. The office staff is committed to a philosophy that allows for the full integration and participation of a person with a disability or disabilities in campus life.
As a new student with a disability, you must register with SDS to receive SDS services and accommodations. You can either register by downloading the intake form below or by calling (313) 577-1851 and making an intake appointment with a learning specialist.

Documentation Required
Before your appointment with an SDS counselor, be sure to have your intake package filled out and all documentation in order. You must have the following documentation provided by your health care provider, physician or medical professional on file to receive SDS services:
- Name and credentials of the evaluator
- Where and when you were seen and for what purpose
- A summary of the diagnostic interview
- A description of the methods of evaluation (tests performed, procedures used, etc.)
- The result or findings of the evaluation (test scores or clinically significant observations of the evaluator)
- A specific ICD-9-CM or DSM-IV diagnosis
- A description of your current level of functioning and any substantial physical, psychological or emotional limitations associated with the impairment underlying the dianosis
- Any relevant history
- Prognosis
- Recommendations for accommodations

Meeting with a Counselor
You have two choices upon scheduling an appointment with an SDS counselor to review for your accommodation options.
- You may choose to have your documentation sent or faxed directly to the SDS office, 313-577-4898. You should follow up with a telephone call to confirm if SDS received your documentation and to schedule an appointment with an SDS counselor, 313-577-1851.
- You can request the documentation be given or sent directly to you and upon receiving it, you are then encouraged to telephone the SDS office to schedule an appointment to speak with an SDS counselor to review your accommodation options.
